GUIDELINES FOR DOWNLOADING AN APPLICATION

We recommend that you obtain an official announcement and application by coming in person to our office. In this way you might find other information which could be helpful in your pursuit of employment.

If you are unable to come to this office, or are approaching the deadline for exam filing, you may download an application from this site.

NOTE: You will require the Adobe Acrobat Reader to view and print application forms

Your application must:

  1. Include CSX-1 and CSX 2.1 forms.
  2. Be printed on an acceptable-quality white paper (min. 20 lb.)
  3. Be printed full-size and in proper orientation (Landscape or Portrait)
  4. Be printed clearly (in black, legible, and not faded)
  5. Have pages printed separately (not two-sided)
  6. Have pages stapled in the upper left-hand corner
If any of the above are not complied with, your down-loaded application may be rejected.

IMPORTANT:

Use the "Print" button in the Adobe Acrobat Reader menu bar to print the form. Do not use the browser's print button.

(You must download and complete both of these forms)

PRINTING FORMS: The CSX-1 form must be printed in Landscape orientation, and the CSX 2.1 form must be printed in Portrait orientation		Download CSX-1 and CSX2.1

The following forms may or may not apply. You should download and read each one to determine if they are necessary. If so, complete, print and mail with Application.

STUDENT LOAN
(Only necessary if you have a NYS loan)

PRINTING FORM: This form must be printed in Portrait orientation		Download Student Loan
TRANSCRIPT REQUEST
(Only necessary if required for exam and not already on file)
This form must be sent to the Registrar of your school. The College must send an official transcript directly to the Civil Service Commission. Once received, the official transcript will be kept on file for all future exams.
