GENERAL STATEMENT OF DUTIES
Plans and implements an outreach program offering health care and/or substance abuse information, services and/or counseling to individuals and communities; performs related duties as required.

COMPLEXITY OF DUTIES
Under direct supervision, the duties involve the use of independent judgment in implementing a community health care and/or substance abuse outreach program offering information, services and counseling.

TYPICAL DUTIES
*1.Promotes community health care screening and/or substance abuse outreach programs.
*2.Addresses target audience in religious and community organizations conveying the need for medical and/or substance abuse screening.
*3.Communicates, establishes, and maintains relationships with private and public health care agencies disseminating health care and/or substance abuse messages.
*4.Interviews, screens and motivates people to obtain treatment in a comprehensive medical and/or substance abuse program.
*5.Assists patients in obtaining access to entitlement programs, financial services, lab testing, preoperative testing and other services.
*6.Collects patient information.
7.Maintains records and prepares reports.

FULL PERFORMANCE KNOWLEDGES, SKILLS, AND ABILITIES
1.Knowledge of methods and procedures used in the treatment and prevention of designated health problem or condition.
2.Knowledge of the techniques of interviewing, and health and substance abuse counseling.
3.Knowledge of community groups and organizations.
4.Ability to express oneself effectively, both orally and in writing.
5.Ability to learn about the various substance abuse, health care and medical services offered by the NUMC.
6.Ability to establish and maintain rapport with clients and potential clients who may be under physical and/or emotional strain.
7.Ability to establish and maintain effective working relationships with associates, private and public agencies, and community groups.

MINIMUM QUALIFICATIONS
Training and Experience
Graduation from high school
and
One year of satisfactory experience advising clients regarding health care and/or substance abuse.
